To ensure a smooth check-in process and timely start to your appointment, we ask that all new patient forms be completed at least 48 hours prior to your scheduled visit. If the forms are not submitted in advance, there may be a delay in your appointment time, as we are unable to begin your visit until all required documentation is complete.
